Eternity Homecare is a long standing personal care service agency network provider of MHS Health Wisconsin (MHS). MHS is a managed care organization operating in all 72 Wisconsin counties, providing Medicaid (BadgerCare Plus and SSI) and Medicare Advantage (Special Needs Plan) coverage. Their provider network includes over 14,000 clinicians and approximately 120 hospitals.

Key details about MHS Health Wisconsin's network and care coordination include:

• Provider Network: The network consists of primary care providers (PCPs), behavioral health providers, and specialists who coordinate with MHS to provide in-network care.

• Care Coordination: MHS Integrated Care Teams work to coordinate services for members with complex medical or social needs. This includes care management, disease coaching (asthma, diabetes, COPD), and helping members find doctors or schedule appointments.

• Member Services & Support: MHS offers a 24-hour Nurse Advice Line (1-800-280-2348) and, in some cases, provides pre-programmed cell phones for eligible members.

• Transportation: MHS coordinates with MTM to provide Non-Emergency Medical Transportation (NEMT) to covered appointments for members.

• Member Choice: Members can select their own primary care provider (PCP) from the MHS network. If a member does not select one within 30 days of enrollment, MHS will assign one.
