ForwardHealth is the umbrella system in Wisconsin that brings together Department of Health Services (DHS) health care and nutritional assistance programs, including Medicaid and BadgerCare Plus. As a provider network, it acts as a comprehensive resource for coordinating care, verifying insurance, and managing benefits across the state.

• Member Services & Support: Members can use their ForwardHealth card to access services, while a dedicated member service line (1-800-362-3002) is available for questions about benefits.

• Provider Services: The network connects providers with tools for billing, prior authorization, and enrollment (1-800-947-9627).
