Eternity Homecare is a network provider of Independent Care Health Plan (iCare) is a Wisconsin-based managed care organization that partners with a wide network of doctors, specialists, hospitals, and clinics throughout the state. As a subsidiary of Humana, iCare serves members through Medicaid SSI, BadgerCare Plus, and Medicare-Medicaid integrated plans (Family Care Partnership).

Key Information for iCare Members & Providers

  • Provider Network & Search: iCare includes a broad range of health systems, including Aurora, Ascension Wisconsin, Advocate Health, Children’s Hospital, ProHealth, UW Health, and others. Members can find a provider via the official iCare Provider Search tool.

  • Coverage Area: iCare services primarily focus on southeastern Wisconsin and other regions, specializing in care for people with disabilities, special needs, and the elderly.

  • Care Coordination: iCare provides care management teams to coordinate medical, social, and behavioral health services.

  • Dental & Vision: Dental services are partnered with DentaQuest (specifically for Kenosha, Milwaukee, Ozaukee, Racine, Washington, and Waukesha counties). Vision care is managed through National Vision Administrators (NVA).
